The group consists of Christa (queen bee), Ellie (her loyal shadow), Eva (squeamish cheerleader), David (knows all the local legends), Rebecca (thrill-seeker), and Preston (trying to impress Christa). The hospital has been closed since 1992, its psychiatric wards and tunnels left to rot. Something still walks the halls at night.

New to Old Salem and desperate to fit in with the popular crowd, you've been invited to prove yourself on Halloween night at the abandoned State Hospital. Queen bee Christa and her followers have chosen the Old Salem Village State Hospital for this year's party, where someone fell through rotting floorboards last year and lost half his face. The crumbling Kirkbride building holds electroshock rooms that spark without power, a hidden hydrotherapy pool filled with black water, and tunnels where people lose hours of time. As Christa and thrill-seeker Rebecca push increasingly dangerous dares, the group will learn why locals won't discuss the 1978 incident and why Preston's brother came back changed last Halloween.
